Our goal was to explore self‐care practices among men who have sex with men in the context of Mpox in Brazil. This study used qualitative research methods, including interviews and thematic analysis, to collect and analyze data from male participants across the Brazilian territory. The narratives unveil men's perspectives on self‐care, risk reduction, and health beliefs during the Mpox pandemic. Our findings highlight a multifaceted approach to self‐care among men, encompassing hygiene, physical contact management, mask usage, skin lesion vigilance, and (...) adherence to official guidelines. Men's attitudes toward sexual behaviors emphasize the importance of reducing sexual partners, practicing safe sex, and combating misinformation through accurate information dissemination. The development of these behaviors and self‐care practices can be facilitated by nurses guided by Dorothea Orem's Self‐Care Theory, supported by patient‐centered care, with strategies to address and confront the stigma associated with the disease and provide emotional support. Thus, the study underscores the pivotal role of self‐care in mitigating infection risks, especially in the context of emerging infectious diseases. It acknowledges the impact of socio‐cultural factors and healthcare policies on men's preventive measures. However, it also recognizes limitations, such as potential bias due to stigma concerns and a nonrepresentative sample. Ultimately, the research advocates for tailored education, promotion of gender equity, and healthcare empowerment to effectively manage health risks in such contexts. (shrink)

ObjectiveThis study aims to analyze sociohistorically how the normative patterns of hegemonic masculinity produced impacts on men’s health/mental health in the context of the COVID-19 pandemic.MethodsA qualitative study from a socio-historical perspective was conducted with 50 men based on an online survey. A semistructured form was applied. The data were analyzed by the Collective Subject Discourse method, interpreted in the light of the context of epidemic disease and hegemonic masculinity.ResultsThe experience of the pandemic exposed the normative patterns of masculinities from (...) the consummation of acts representative of the pandemic context, which incited men to deny the existence of COVID-19 disease and to delay the understanding and adoption of measures to protect and control COVID-19. As a repercussion, men presented conflicts in the regulation of emotions; presented emotional suppression; were more reactive; felt threatened regarding the loss of the role of family provider, virility; and revealed a sense of invulnerability, added to the weakening of self-care.ConclusionThe discourse revealed that the men’s behaviors are consistent with the characteristics of hegemonic masculinity, but express signs of recognition that this behavior causes harm to themselves and their health. A kind of “ruthless reductionism” characterized the experimental practices of the first two decades of molecular and cellular cognition. More recently, new research tools have expanded experimental practices in this field, enabling researchers to image and manipulate individual molecular mechanisms in behaving organisms with an unprecedented temporal, sub-cellular, cellular, and even circuit-wide specificity. These tools dramatically expand the range and reach of experiments in MCC, and in doing so they may help us transcend the worn-out and counterproductive debates about “reductionism” (...) and “emergence” that divide neuroscientists and philosophers alike. We describe examples of these new tools and illustrate their practical power by presenting an exemplary recent case of MCC research using them. From these tools and results, we provide an initial sketch of a new image of the behaving organism in its full causal-interactive complexity, with its molecules, cells, and circuits combined within the single system that it is. This new image stands in opposition to the traditional “levels” image of the behaving organism, and even the initial sketch we provide of it here offers hope for avoiding the dreary metaphysical debates about “emergence” and “downward causation,” and even the reduction vs. anti-reduction dispute, all dependent upon the familiar “levels” image. À quoi bon la pensée? Pour de nombreux chercheurs, inspirés par les théories évolutionnistes, la pensée réfléchie est utile à notre espèce. Elle lui confère des avantages importants et contribue à son succès reproductif. Pourtant ses avantages ne sont pas si évidents. La pensée ne figure ni dans les mécanismes de l'évolution qui ont façonné la vie, ni parmi les procédés dont se servent la plupart des organismes pour s'y maintenir. Dans Évolution et rationalité, Ronald de Sousa montre que, (...) pour comprendre les avantages de la pensée réfléchie, il faut changer nos présupposés relatifs à la rationalité et reconnaître la multiplicité de ses dimensions. La rationalité ne nous sert pas seulement à avoir des croyances vraies et à agir de manière efficace dans un environnement changeant. Elle nous aide à gérer nos plaisirs et nos peines dans le temps et nous permet d'envisager des situations qui n'existent pas. En général, le statut de la rationalité comme ensemble de capacités ayant évolué dans le cours de l'histoire naturelle de notre espèce, passant des tropismes aux désirs et de la détection à la représentation, n'a pas été pris suffisamment au sérieux. Suivre ce processus d'évolution de la rationalité permet de comprendre comment elle est devenue une norme, mais aussi pourquoi il lui arrive aussi souvent d'échouer. Ronald de Sousa a profondément modifié nos façons de concevoir les émotions. Avec Évolution et rationalité, il pourrait avoir une influence aussi décisive sur nos façons de concevoir la rationalité. We present the system SUBSEXPL used for simulating and comparing explicit substitutions calculi. The system allows the manipulation of expressions of the λ-calculus and of three different styles of explicit substitutions: the λσ, the λse and the suspension calculus. A variation of the suspension calculus, which allows for combination of steps of β-contraction is included too. Implementations of the η-reduction are provided for each style. Other explicit substitutions calculi can be easily incorporated into the system due to its modular structure. (...) The uses of the system include: the visualization of the contractions of the λ-calculus in de Bruijn notation, and of guided one-step reductions as well as normalization via each of the associated substitution calculi. Many useful facilities are included: reductions can be easily recorded as text files, LATEX outputs can be generated and several examples for dealing with arithmetic operations and computational operators such as conditionals and repetitions in the λ-calculus are available. The system can be executed over Emacs using the x-symbol mode in such a way that λ-terms and terms of the explicit substitutions calculi are represented in its natural syntax avoiding the necessity of repeatedly generating LATEX outputs. The system has been of great help for systematically comparing explicit substitutions calculi, as well as for understanding properties of explicit substitutions such as the Preservation of Strong Normalization. In addition, it has been used for teaching basic properties of the λ-calculus such as: computational adequacy, the usefulness of de Bruijn's notation and of making explicit substitutions in real implementations. The precarious rights of senior citizens, especially those who are highly educated and who are expected to counsel and guide the younger generations, has stimulated the creation internationally of advocacy associations and opinion leader groups. The strength of these groups, however, varies from country to country. In some countries, they are supported and are the focus of intense interest; in others, they are practically ignored. For this is reason we believe that the creation of a network of all these associations (...) is essential. The proposed network would act as a support for the already-existing policies of the United Nations’ High Commission for Human Rights, of independent experts, and of the Global Alliance for the Rights of Older People. All three have long ago recommended the creation of a recognized instrument for uniting presently scattered efforts. The proposed network, therefore, will seek to promote the international exchange of relevant expertise, and it will reinforce the commitments and actions that single countries are currently taking to meet these objectives. For example, informative public events can be organised to promote particular support initiatives and to provide an opportunity for new members of the network to be presented. The network will promote health for senior citizens, disease prevention, senior mobility, safe free time for seniors, alimentary education, protection against new risks and dangers, as well as equity in the services necessary for seniors to adopt new information and communication technologies. In the case of retired academic members, the network will promote equality with respect to continuing use of digital technologies (particularly email), continuing access to research libraries, and the guaranteed ability for seniors to fund their own research programs and to deliver free seminars. Research involving transgender and gender diverse people (TGD) increased in the last years, mostly concerning healthcare associated to this population. Few studies dedicated their analysis to the impact of parental support on transgender people, even though this is an important aspect in creating a safe environment on which these individuals can build their identity. In addition, the link between family support, TGD identity and homelessness is not completely established. Thus, due to the specificities of the family context of TGD individuals, (...) the aim of this study is to investigate the association between family support and TGD in different moments of the process of gender affirmation. In addition, this study also aims to explore the relationship between a lack of social support and low self-esteem, home abandonment, and dwelling in the street. The survey was designed based on the TransPULSE project and was made available in electronic format. The sample was constituted of 423 TGD residents in two Brazilian states. A Structural Equation Model analysis suggested that the impact of gender affirmation status on homelessness was mediated by parental support, through self-esteem, and the need to move from home. The association between the status of the gender affirmation procedures, family support and self-esteem was significant and indicated that the further TGD individuals advanced in gender affirmation, the more self-esteem and family support they would have. The association between family support and self-esteem indicated that family support was associated with higher self-esteem. Low family support was associated with the willingness to move from home due to one’s TGD status and there was also a significant correlation between low self-esteem and the willingness to move from home due to one’s TGD status. Finally, homelessness was associated with the willingness to move with a large effect size. Limitations include the sample that was constituted by individuals with Internet access and who had more contact with TGD communities. The findings indicate directions for interventions involving TGD people and their families, considering the parental relationship as a critical variable to improve TGD quality of life in the process of gender affirmation. La parentalité est, avant tout, liée à un processus qui comporte des risques psychiques particuliers face aux événements de vie. Identifier ces enjeux dans la relation parent-enfant et comprendre ce qui est sollicité pour le sujet face à des événements nécessite de remodeler les alliances et de trouver un nouvel équilibre psychique. Dans le cadre (...) d’une recherche clinique, les auteures, impliquées dans un dispositif d’écoute psychanalytique de la parentalité, proposent une discussion quant aux repères cliniques des enjeux qui sous-tendent la relation parent-enfant. Ce questionnement est discuté par deux vignettes cliniques, celle de Maria qui, après son accouchement, expérimente des vécus corporels primitifs projetés sur son bébé et celle de Camila, qui commence son analyse après la tentative de suicide de sa fille. Dans l’ensemble de ces vignettes, on observe une réorganisation de la dynamique psychique et une possibilité de symboliser davantage les modes d’alliance de la dyade parent-enfant. (shrink)

Automatized scalable healthcare support solutions allow real-time 24/7 health monitoring of patients, prioritizing medical treatment according to health conditions, reducing medical appointments in clinics and hospitals, and enabling easy exchange of information among healthcare professionals. With recent health safety guidelines due to the COVID-19 pandemic, protecting the elderly has become imperative. However, state-of-the-art health wearable device platforms present limitations in hardware, parameter estimation algorithms, and software architecture. This paper proposes a complete framework for health systems composed of multi-sensor wearable health (...) devices, high-resolution parameter estimation, and real-time monitoring applications. The framework is appropriate for real-time monitoring of elderly patients' health without physical contact with healthcare professionals, maintaining safety standards. The hardware includes sensors for monitoring steps, pulse oximetry, heart rate, and temperature using low-power wireless communication. In terms of parameter estimation, the embedded circuit uses high-resolution signal processing algorithms that result in an improved measure of the HR. The proposed high-resolution signal processing-based approach outperforms state-of-the-art HR estimation measurements using the photoplethysmography sensor. (shrink)

Le problème métaphysique central en philosophie de l’esprit concerne la relation entre l’esprit et le corps des agents. Quand on tente d’expliquer, par exemple, le rapport entre les pensées et les actions humaines, on est alors immédiatement confronté avec la difficulté, apparemment insurmontable, d’expliquer la causalité mentale. On doit répondre à la question : nos états de pensée causent-ils effectivement ce que l’on fait? Bien sûr, nos croyances, nos intentions et nos désirs sont à la base de notre comportement, dira (...) le sens commun et une longue tradition philosophique. Cependant, certains philosophes ont récemment manifesté des doutes à propos du rôle réel de nos pensées sur nos actions. Pour contrer ce scepticisme, il nous faut expliquer comment des états de pensée peuvent exercer leur rôle causal dans un monde fondamentalement physique. Je vais donner ici les raisons pour lesquelles je crois que les états de pensée sont indispensables à l’explication complète de nos actions. Pour ce faire, je procéderai à une analyse approfondie de la relation entre la causalité mentale ou intentionnelle et la causalité physique. Je mettrai en perspective deux approches importantes sur la causalité mentale : celle de John Searle, qui défend la réalité des états mentaux et semble résister au réductionnisme, et celle de Jaegwon Kim, qui montre le péril auquel l’approche de Searle est exposée : la surdétermination causale! Selon Searle, le mental est un trait du monde physique . Aux yeux de Kim cette stratification en niveaux n’apporte aucune solution au problème de la causalité mentale. Kim a-t-il raison? (shrink)

Background: Discussing treatment risks has become increasingly important in medical communication. Still, despite regulations, physicians must decide how much and what kind of information to present. Objective: To investigate patients’ preference for information about a small risk of a complication of colonoscopy, and whether medical and personal factors contribute to such preference. To propose a disclosure policy related to our results. Design: Vignettes study. Setting: Department of Gastroenterology, Academic Medical Centre, the Netherlands. Patients: 810 consecutive colonoscopy patients. Intervention: A home-sent (...) questionnaire containing three vignettes. Vignettes varied in the indication for colonoscopy, complication severity and level of risk. Patients were invited to indicate their wish to be informed and the importance of such information. In addition, sociodemograhic, illness-related and psychological characteristics were assessed. Main outcome measurements: Wish to be informed and importance of information. Results: Of 810 questionnaires, 68% were returned. Patients generally wished to be informed about low-risk complications, regardless of the indication for colonoscopy or the severity of the complication. The level of risk did matter, though (OR = 2.48, SE = 0.28, p = 0.001). The information was considered less important if done for population screening purposes or diagnosis of colon cancer, if the complication was less severe (bleeding) and if the risk was smaller (0.01% and 0.1%). Patients’ information preference was also related to age, mood and coping style. Limitations: Difficulty of vignettes. Conclusions: Patients generally wish to be informed about all possible risks. However, this might become uninformative. A stepwise approach is suggested. (shrink)

Current UK legislation is impacting upon the feasibility and cost-effectiveness of medical record-based research aimed at benefiting the NHS and the public heath. Whereas previous commentators have focused on the Data Protection Act 1998, the Health and Social Care Act 2001 is the key legislation for public health researchers wishing to access medical records without written consent. The Act requires researchers to apply to the Patient Information Advisory Group for permission to access medical records without written permission. We present a (...) case study of the work required to obtain the necessary permissions from PIAG in order to conduct a large scale public health research project. In our experience it took eight months to receive permission to access basic identifying information on individuals registered at general practices, and a decision on whether we could access clinical information in medical records without consent took 18 months. Such delays pose near insurmountable difficulties to grant funded research, and in our case £560 000 of public and charitable money was spent on research staff while a large part of their work was prohibited until the third year of a three year grant. We conclude by arguing that many of the current problems could be avoided by returning PIAG’s responsibilities to research ethics committees, and by allowing “opt-out” consent for many public health research projects. O autor analisa a crítica que fez Wittgenstein aos fundamentos de Matemática na dupla fase do seu pensamento lógico e filosófico. Começa por situá-lo em relação às 3 Escolas que discutiam sobre a fundamentação lógica da matemática: o logicismo, o intuicionismo e o formalismo. Na 1.a fase do Tractatus, vê-se que Wittgenstein é logicista. Mas é original porque não deriva a aritmética do cálculo de classes, como fazia Russell, mas do cálculo proposicional, que generaliza. Considera a matemática como um simples (...) "método lógico". Por isso define o Número como "expoente duma função proposicional"! Mas neste caso trata-se implicitamente, duma nova extensão analógica de princípios e conceitos da aritmética à lógica pura. Por exemplo: noções de número, operação, exponenciação, etc. E se Wittgenstein quer criar, por analogia, números lógicos (símbolos puros), então tem de fazer uma interpretação semântica para passar da lógica formal pura para a aritmética! Só admite o infinito potencial na matemática. Infere-se, pois, que na l.a fase do Tractatus, Wittgenstein, é nominalista e finista. Na 2a fase das Notas (PB ou Remarks) que são manuscritos e fotocópias, (entre 1927 e 1944) Wittgenstein foi variando de pensamento. Agora julga que a matemática se funda a si mesma, como autónoma de lógica pura. Descobre que para além dos "Símbolos" há relações para os "significados" e "usos". E a matemática pode ser teórica e aplicada aos cálculos das ciências. Mas pela sua crítica intuicionista rejeita o valor da teoria dos números irracionais de Dedekind, do teorema e processo da diagonal com que Cantor prova a teoria dos números transfinitos... Wittgenstein aborda ainda os célebres teoremas de Fermat e de Gödel, quando analisa as proposições indecidíveis dos Principia mathematica (de Russell-Whitehead). O autor mostra, no artigo, que Wittgenstein não tem razão na sua crítica pouco profunda ao conceito de número e suas generalizações. .. He discovers beyond "symbols" there are relations for "meanings" and "uses". And mathematics can be theoretical and applicable to the calculus of sciences. But by his intuitional criticism he can reject the value of the theory of irracional numbers of Dedekind, of the theorem and the process of the diagonal, with wich Cantor proves the theory of transfinit numbers... Wittgenstein refers still the famous theorems of Fermat and Godel when he examines the indecidable propositions of the Prinvipia Mathemativa (of Russell-Whitehead). In this article the author shows that Wittgenstein is not correct in his not too profound criticism of the concept of numbers and its generalizations. Comment envisager les concepts de peine et de sanction dans le contexte du droit et de la morale? C'est pour réfléchir à cette question que se sont réunis, à l'occasion de la Journée d'études du 13 novembre 2007 intitulée : « Entre droit et morale : la finalité de la peine », des juristes de renom et des historiens des idées. Ceux-ci étaient invités par l'équipe du Centre de Recherches Pluridisciplinaires et Multilingues de l'Université Paris Ouest Nanterre la Défense. Les (...) travaux réunis exposent en particulier le statut de la sanction pénale dans différents Etats européens, de l'Antiquité grecque et romaine à la période moderne. Est particulièrement mis en valeur le tournant pris par le droit pénal à la fin du 18ème, début du 19èmesiècle, à partir des conceptions innovantes d'un fonctionnaire de l'Etat autrichien en Lombardie, Beccaria, qui a repensé le droit de punir de l'Ancien Régime, puis de la philosophie de la peine développée en Angleterre par Bentham. Pour terminer, l'ouvrage présente les réflexions de Nietzsche sur la faute dont l'originalité se résume en ces termes : comment « surveiller sans punir »? (shrink)
